Standards of practice in nursing: is it important?

Standards of practice in nursing: is it important?
By: Dantes, Michelle M.
Standards of practice (standards of care) are guidelines used to determine what a nurse should or should not do. Standards may be defined as “a benchmark of achievement which is based on a desired level of excellence”. Standards of care (SOC’s) measure the degree of excellence in nursing care and describe a competent level of nursing care.

In recent studies conducted, results shows that many nurses are face with lawsuits. It’s either negligence or malpractice. Most of these nurses appealed innocent or not guilty. Of course, who would want to accept the guilty charged that would mean suspension or worse, a revocation of your license as a nurse. 4 years or more of hardship to finish nursing, one single mistake can take it all away. That is why the ANA and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) have established nationally recognized standards of care.  These standards can hold nurses accountable, regardless of their area or state of practice. Standards of care can be found at a national, state and local level. All levels of expertise, from new graduate to experience nurse, should be able to meet these expectations of practice. Nurses have been held accountable for error in medication administration, failure to protect the patient from harm and failing to monitor the equipment for any malfunction which eventually can cause harm to patient. Any nurses who have been found incompetent or having lack of skills in promoting wellness and rendering care to patient is found liable in malpractice. And a nurse who does not meet accepted standards of care is at risk for being found negligent. Nurses in short doesn’t have a choice, the nature of nursing profession requires perfection. It requires honest devotion to fulfill your duties and responsibilities. Admit it or not, But nurses made mistakes sometimes or should I say most of the time. Nurses are humans after all.  No matter how hard they try not to make mistakes, mistakes are inevitable but are preventable. So no matter what standards of practice they established, I believed it is a duty of the nurse to make a right judgment call. A nurse must use her/his initiative at all times. Nurse must weigh the consequences of her actions all the time.He or she must keep in mind that the sphere of a nurse's accountability is to the client, to ensure safety and promote health. Next is the employing agency or the hospital and lastly is the profession, where in nurse took an oath not to do harm to the patient. Nurse is mostly responsible and accountable for the quality of nursing care given to clients. The single most important protective strategy for the nurse is to be a knowledgeable and safe practitioner of nursing and to meet the standards of care with all patients. A standard of practice is there to serves as an outlines of what is expected. It promotes guidance and directs professional nursing practice appropriately thus making fewer problems to occur.

Alexander the great and his exemplary leadership.

Alexander the great and his exemplary leadership.
By: Dantes, Michelle M.
How do we define a good leadership? When can we say that he/she is a good leader?
Most of the time, we often heard that leaders are born. They usually came from rich and powerful families or some may call them “royalties”. They are well known individuals. But others believe that to become a leader, one should possess those special traits of a leader.
There are lots of leaders known today. Most of the good leaders were part of the history. In which their greatness will be forever remembered. One of the famous and highly celebrated character known for his leadership was Alexander the great.  He became king at the age of 20 and engaged his self in an amazing 11 years of journey of conquest and fought over 70 battles without a single loss.  With a mere of 32 years life span, he was able to conquer half of the world (India to Egypt).

A good leader must have a vision which empowers his people. A visionary leader should be able to tap into the hopes and dreams of people and create a destination that not only inspires but also challenges them to be better person. It’s not how to accomplish the vision but instead to create environment in which people are free to explore, innovate and experiment. The basis of Alexander’s success as a leader was his ability to inspire his men to think great things and then accomplish them with passion, enthusiasm and commitment. His vision was called “the brotherhood of man” in which he believed would bring together the different cultures into one.

Another thing about leaders is that their focus is their people. They demonstrate fairness towards their followers. In Alexander’s conquest, often times he encountered hard ship, there was a time (after a battle against king Darius) that Alexander and his men were nearly dead from thirst. His men having the respect for Alexander’s authority offered him a bag of water, although his mouth was so dry, he gave back the helmet with his thanks.  He then explained that the water wasn’t enough for everyone and if he drinks, others will faint.  This will only show that a good leader doesn’t seek comfort for his self. A good leader must think for his people first.

Another ability of a good leader is to be a creative thinker or the ability to innovate things when the situation calls for it. One of the stories that made Alexander more popular was when he accepted the challenge of the Gordian knot.  The Gordian knot was tied together with the yoke of an ancient chariot and was believed that whoever could undo the knot would become the master of the world. Many people were believed also tried to undo the knot but only Alexander was able to do so. He just pulled out his sword and chopped through the knot instead of involving himself in its mysterious entanglements.

And lastly, a good leader must communicate well to his people. Alexander was able to convey the purpose of their journey. He was able to determine the strength and weaknesses of his people thus maintained the effectiveness of the group. He communicates to his fellowmen not as a king but as a leader. He seeks information and suggestion to his people.

There are still a lot of other things and abilities that a good leader must have. Those were just some of which I believed Alexander the great possessed.

A deeper understanding of Classical organization theory

A deeper understanding of Classical organization theory
By: Dantes, Michelle M.
When talking about management, what comes first in your mind? I guess that would be business right.
For so many years, management often co relates with business. A traditional belief says that in business, a proper management is indeed a key variable in success. But what is management?  Management is a process of coordinating and supervising personnel and resources to accomplish organizational goals. It comprises planning, organizing, staffing, directing and controlling.
In management, it utilizes theories, the same as of the others. A lot of theories regarding management have arises. But I am much convince with the theory made by Henry Fayol (father of management process) entitled classical organization theory, wherein he proposed 14 principles of management.
  1. Unity of command- Each employee has one and only one boss. Which I do believe that having a single leader tends to be less complicated and confusing. Decisions can be made quickly and informally in emergency situations. When there are several owners- managers looking after the business, leadership responsibilities may become diffuses leading to sometimes slow, ineffective actions.
  2. Unity of direction- one head and one plan for a group having the same objective. This can harbor cooperation with the members. It is the knowing that you are headed with the same direction with nothing more and nothing less to expect. This can prevent unnecessary stress or pressure to both manager and subordinates.
  3. Subordination of individual interest to general interest- it means that there should be no conflict of interest between individuals ambition and the well being of the organization as a whole. This principle requires a firm but fair hand from superior who should set a good example. It requires constant supervision. Human as we are, sometimes we tend to do thing which is against from our principle. We develop " hidden agenda" for our personal good.
  4. Division of labor-it is making sure that work is equally distributed among the group. Group projects are designed to help you improve your ability to lead and work as part of a team. One of the most common sources of conflict is the improper division of work. One member can be loaded with work and the other may not. And that may end up in misunderstanding thus making your group slow on progress.
  5. Esprit de corps- it’s the Harmony or cohesion among personnel. It's a great source of strength in the organization.  When a group works in harmony, results would be very productive and fewer interruptions will happen.
  6. Respondeat superior- the term is referring to the request of the court that you answer for any liability you may have even when you had an agent acting on your behalf.. In nursing it refers to the accountability of nurses in delegating work to other personnel.  You may not be held responsible for the mistake of your people but you are accountable for it. It means you are the one who is answerable if your agent acts in such a way that is not deemed legal, or involves intentional tort. But it does not mean your agent is relieved any liability in the matter.
  7. Centralization-it is where the decision is made from top to bottom. It has an order. In business the one who likely makes the decision is the manager, whatever the decision, members must follow it. Or on the other way, all the important decision and actions proposed at the lower level are subjected to the approval of top management.
  8. Tenure- it is defined as a length of service or term of office that a worker have. Tenure in short provides direction a worker needs to take and follow. It is the legality of the contract you made with the company.
  9. Authority- it was defined by Henry Fayol as the “the right to give orders and the power to exact obedience”. It is the distinguishing advantage of a leader.  A leader has the authority to make things happen. It is the basis in which he can issue task, responsibilities and obligations to the followers.
  10. Justice and equity-Equality of treatment. No favoritism is allowed.  Favoritism is a destructive trait.  Favoritism results in making a decision based not upon the merits of the matter at hand, but upon some other basis. It can lead to false decision and can affect the group performance.
  11. Span of control- it refers to number of subordinates a supervisor has.  The supervisors control only suited for those who are under him. His legitimacy will be effective only for those who are under him.
  12. Remuneration-Employees receive fair payment for services.  It is believed that giving a fair salary encourages keenness by rewarding a well directed effort.  I also believed that giving a fair salary is the most effective way on retaining and motivating employees.  As part of human behavior, we usually respond to which we know our effort would be rewarded.
  13. Channel of communication – it refers to the way this information flows within the organization and with other organizations. And since we already know that this type of management uses centralization, it means the decision is made by the manager.
  14. Chain of command- a command running from top to bottom of the organization, it is just the same with the centralization.

This theory is the most organized theory in management. It gives specific direction and job description on which the leader must do to attain optimum productivity of the group.  The 14 principles not only help the leader but also it safeguards the role of the member.
So if one is planning to have a business establish, then I highly recommend following this theory.
